Individualized Online Care Planning Tool for Family Members of Persons With Dementia

What Is This Study About?

This study will test an online care planning tool, Care to Plan (CtP). The tool is designed to offer a more efficient, user-directed process to link dementia caregivers to the services that may be most appropriate for them, based on their needs, the needs of their relatives living with Alzheimer's disease or a related dementia (ADRD), and other factors. Family caregivers of persons with ADRD who seek services in the Riverside Health System in the state of Virginia will be randomly assigned to either receive the CtP intervention or usual care. Participants of the CtP intervention will receive help to enroll in a recommended support service, if desired.

Do I Qualify To Participate in This Study?

Minimum Age: 21 Years

Must have:

Care Recipient:

  • Physician diagnosis of Alzheimer's disease or a related dementia


  • English-speaking
  • Self-identifies as someone who provides support and care to a person with dementia
  • Willingness to use Care to Plan
  • Resides in one of four Riverside health regions in Virginia (based on zip code)

Must NOT have:

  • History of a serious mental health disorder, the symptoms of which have gotten worse in the last six months OR not receiving steady, ongoing pharmacological or other treatment for these symptoms
